May 11, 2014 of Cheektowaga, NY; Beloved wife of Leonard E. Kozlowski. Dearest mother of Carl (Sarina), Julie and Mary Ellen Kozlowski. Dear grandmother of Bianca and Mariah. Daughter of the late Stephen and Florence (nee Rogacki) Kubiak. Sister of the late Stephen Jerome (Elaine) Kubiak. Aunt of April (Richard) Bickart, Stephen (April) Kubiak and Edward Kubiak. Family will be present to receive relatives and friends on Tuesday 2-4 & 7-9 p.m. at the MELVIN J. SLIWINSKI FUNERAL HOME 5090 Transit Road (south of Como Park Blvd.). A Mass of Christian Burial will be celebrated on Wednesday in Resurrection Church 130 Como Park Blvd. at 10:00 a.m. (please assemble at church) Entombment St. Stanislaus Cemetery. Flowers gratefully declined. If desired, contributions may be made to either Hospice Buffalo 225 Como Park Blvd. Cheektowaga, NY 14227 or The ALS Association 1275 K Street NW, Suite 250 Washington, DC 20005.
